Presentation Title: ‘Integrum Aged Care+’ Developing a world class peri-end-of-life service
Daryl has worked in healthcare for 20 years, with diverse clinical experience in General Practice, Emergency and Urgent Care. He is a vocationally registered GP in Australia and the UK and is currently completing his MHA through Monash University. Daryl has developed a particular focus in the area of community based care for the last ten years. During this time he has been involved as a clinician and administrator in various models of service provision within both the public and private service systems. He is a Board Member of the Hospital in the Home Society of Australasia and is the convener of the 2019 HITH Society Annual Scientific Meeting. He was the recipient of an Endeavour Executive Award in 2011 to study innovative models of diabetes care in North London. In July 2015, Daryl moved with his family to Perth to take up the role of Medical Director of Home Hospital with Silver Chain, which is the largest Hospital in the Home in Australia. Subsequently, he was appointed National Medical Director with Silver Chain Group, and is accountable for 50 medical practitioners based in various models of care within Australia. Daryl is the clinical lead of a new program Integrum Aged Care+ that combines chronic disease management and aged care packages in a novel health/social venture aiming to transform the approach to care in the last few years of life.
